वमनं विरेचनं बस्तिः शिरोविरेचनं तथा। रक्तमोक्षणमित्येते पञ्चकर्माणि कीर्तितानि॥
vamanaṁ virecanaṁ bastiḥ śiro-virecanaṁ tathā | rakta-mokṣaṇam ity ete pañca-karmāṇi kīrtitāni ||
— Charaka Samhita, Si. 2/14 — Vamana, Virechana, Basti, Shirovirechana (Nasyam), and Raktamokshana — these are the five karmas of Panchakarma.

The Five Therapies

Vamana (therapeutic emesis) — Eliminates excess Kapha from the respiratory and digestive tract. Particularly effective for asthma, chronic skin conditions, and Kapha-type obesity. Requires skilled supervision and proper Purvakarma (oleation and fomentation) preparation.

Virechana (therapeutic purgation) — The gold standard for Pitta-related disorders including liver disease, chronic skin conditions, inflammatory states, and hormonal imbalance. Medicated ghee is used in the preparatory phase.

Basti (medicated enema) — Described by Charaka as equivalent to "half of all treatment." The most important Panchakarma therapy for Vata disorders — addressing joint disease, neurological conditions, and reproductive health. Two types: Niruha (decoction-based) and Anuvasana (oil-based).

Nasyam (nasal administration) — Clears accumulated Kapha from the head, neck, and sinuses. Profoundly effective for migraine, sinusitis, memory, and mental clarity. The nose is the doorway to the brain in Ayurvedic anatomy.

Raktamokshana (blood purification) — For specific skin conditions and blood-borne disorders. Various methods including leech therapy (Jalaukavacharana) for localised inflammatory conditions.
